Turning 65: Frequently Asked Questions

Do I automatically get Medicare when I turn 65?

Not always. If you’re already receiving Social Security or Railroad Retirement benefits, you’ll usually be enrolled in Medicare Parts A and B automatically. If not, you’ll need to sign up during your Initial Enrollment Period, which starts three months before you turn 65 and ends three months after your birthday month.

Do I need Medicare if I’m still working at 65

Most people apply three months before turning 65 to avoid late penalties or coverage gaps. Your Initial Enrollment Period lasts seven months total — three months before, your birthday month, and three months after.

What does Medicare cost when I turn 65

Part A (Hospital Insurance): Usually free if you or your spouse paid Medicare taxes long enough.

  • Part B (Medical Insurance): Most people pay a standard monthly premium that can increase based on income and adjusts annually. In 2025 the base cost is $185.
  • Additional coverage like Part D (drug plans) or Medicare Advantage plans may have separate costs. A (Hospital Insurance): Usually free if you or your spouse paid Medicare taxes long enough.

What happens if I miss my Medicare enrollment?

You may face late enrollment penalties and delays in coverage. These penalties can last a lifetime, so enrolling on time is important unless you qualify for a Special Enrollment Period (like having employer coverage).
